Useful Business Forms to Download from Omega Accountancy

A selection of useful forms has been provided which can be used by businesses to carry out a variety of tasks. Some downloads are specific to incorporated Companies whilst other forms can be used by any type of business entity. All form downloads are provided completely free of charge and are available for use by both clients of Omega Accountancy and the general public.

Download Companies House Statutory Forms

Form 287 is used for the purposes of changing a company’s registered office address.
Form 225 facilitates the change of a company’s accounting period, its financial year end.
Form 288a is used to appoint a new company director or secretary.
Form 288b is submitted when a director or secretary is to resign from that position in a company.
Form 288c facilitates the changing of details for a company director or secretary, such as their residential address or occupation.
Form 10 records information on the first directors, secretaries and subscribers for the purposes of forming a new company.
Form 12 is the declarant document used as confirmation that Form 10 details are correct.
Form 88(2) is for limited companies who wish to issue of additional shares.
Form 123 is used to increase the authorised share capital in a company.
Form 652 is used to initiate the dissolution of a company and striking off of the Companies House register.
DCA form is used for the submission of financial accounts for non-trading, dormant companies.
NC19 is a sample of a special resolution for changing a company’s name.
NC20 is a sample of a written resolution authorising a company name change.
PR1 Form authenticates any changes made at Companies House relating a to UK company.

CT41G provides information on new company, its principle offices and proposed business activities.
CT600 is the corporation tax return information to accompany computations on a company’s tax liability.

Form P11 is used for the calculation of a business’ PAYE and National Insurance Contributions which should be paid to Revenue and Customs.
Form P32 records remittances to Revenue and Customs in a given tax year.
Form P35 is the year end PAYE and National Insurance Contributions statement for a business.
Form P38(S) can be used for students employed on a temporary basis so that tax does not have to be taken from the salaries.
Form P46 is for where a new employee is unable to provide a P45 from a previous employer this form is used to obtain the appropriate tax code for them.
Form P60 is an end of year summary given to each employee detailing the totals of amounts paid and deducted for a given tax year.
P11D is for all employees earning over £8,500 per year, this form details the taxable benefit of payments made to them.
Form P14 is prepared by employers at the year end and summarises payments and deductions for an individual employee.
P9D is used for recording benefit and expense payments to employees earning less than £8,500 in a given tax year.
P11DX is the Revenue and Customs forms used to apply for a dispensation so that non taxable reimbursements and other tax neutral benefits do not have to be recorded on a P11D form.
P45 is used when an employee leaves a company and provides information to Revenue and Customs, the employee and a future employer on the earning and tax position on cessation.

SA800 is the partnership tax return is used to report the earnings of such entities to Revenue and Customs.
SA355 facilitates and appeal against an imposed charge for the late delivery of a tax return.
SA100 Charity informs Revenue and Customs that all or part of a tax repayment should be given to a designated charity. The form can also be used to set up a gift aid programme.
SA100 individual tax return form where income and capital gains are recorded and reliefs and allowances are claimed.

CWF1 is used to notify the tax authorities that a person has become self employed and what particulars should be disclosed.

VAT 1 is used to register businesses for VAT and provide the required information about the company’s planned taxable turnover.
